2010 Outdoor registration fees & status

Youth (if player is 17 yrs of age or younger):

1. Fill out registration form - select Outdoor 2010 on top of form

2. Sign BOTH pages

3. Attach proof of Date of Birth (copy of OHIP card or birth certificate)

4. Cheque or money order only (payable to Milton Soccer Academy)- check refund policy

5. Mail to address below

Note: your space will be confirmed on or before session starts.

Adults (if player is 18 yrs of age or older)

** These are travelling teams playing in the ESL or NPHSL.

Players on a waiting list may be called until 2 weeks after the session starts. Proof of date of birth is required on or before 1st practice.

No refunds will be given after May 1st for the outdoor season, or after October 1 for the indoor season unless sanctioned by the Registrar and the Treasurer. An administration fee of $30.00 per player will be levied. Refunds will be given only if the uniform is returned personally to the League Convenor, coach or Registrar. Refunds will only be processed when the MSA Refund Form, completed and signed, has been received by the Registrar.

Wait Lists Summer 2010

Please check your wait list status by looking for for your unique moniker. Your moniker is comprised of your child's last initial, first initial, and the last 2 digits of your tel# - so, for example, your child's name is Cristiano Rinaldo with a tel# of 0113491267453746, then your wait list moniker is RC46.

Make sure you check frequently. typically, we experience movement after May 1 and after season start. Even though you may be low on the list, you might be called quickly as others ahead of you drop off the wait list due to alternate arrangements.
