Please note that our governing organization, the Peel Halton Soccer Association, has decided to raise membership fees retro-actively Oct. 1, 2011. Therefore, the fees have been adjusted by $5 per player to cover this increase. Any concerns, please contact the PHSA.

Registration

1. Read schedule info and ensure it fits in with your other activities

2. Read the Refund Policy below

3.1 If you are 18 and over, use this adult registration form and fill out on screen

3.2 If you are under 18, click on the youth registration form and fill out on screen

3.3 No problem if you don't know the OSA Number

4. Print and sign both pages

5. Attach payment (cheque or money order preferred)

5.1 You may post date cheque for Outdoor to January 1, Indoor to September 1

6. If Youth, attach proof of birth date (copy of OHIP card or birth certificate)

7. Mail to, or drop off, at the club address

 

Refund Policy

No refunds will be given after May 1st for the outdoor season, or for the Indoor season, after October 1 (Fall Session) or after January 1 (Winter Session) or the start of the season, whichever comes first,  unless sanctioned by the Registrar and the Treasurer.

An administration fee of $30.00 per player will be levied for any refund request. Refunds will be given only if the uniform is

returned personally to the League Convenor or Registrar. Refunds will only be accepted when the MSA Refund Form,

completed and signed, has been received by the administrator.
